Become a Patient

In order to become a patient of Johnson Health Center, simply call to schedule your first appointment! Our services include adult and pediatric primary care, behavioral health, dental, OB/GYN and pharmacy. 

If you would like to save time at your first appointment, you can fill out the new patient paperwork found below and bring it to the appointment with you. Please be sure to fill out the Release of Records form if you are currently established as a patient at another medical facility.
