Behavioral Health Nurse (LPN or MA)

The Behavioral Health Nurse (LPN or MA) works in conjunction with Behavioral Health providers to provide professional, team based integrative care and nursing support in a clinical setting. Under clinical supervision of the Nursing Director, administrative supervision of the Behavioral Health Director, the Behavioral Health nurse plays a critical role in the delivery of quality, patient- centered integrative care within the context of a federally (HRSA) qualified community health center. (FQHC).

Substance Use Specialist

The Substance Use Specialist (SUS) is responsible for substance use disorder (SUD) referrals, ensuring that referred patients are connected with appropriate services, whether internal or external. The SUS conducts SUD and Office Based Opioid Therapy (OBOT) intakes, new patient appointments, SUD consults, case coordination appointments, provides SUD counseling both individually and in group settings. The SUS collaborates with providers and nursing staff, coordinates as needed, and provides crisis intervention. All duties and responsibilities are performed under the supervision of the Director of Behavioral Health. Bachelor’s degree in psychology, social work, or related field required. Master’s degree in social work or counseling preferred.

Dental Assistant

The Dental Assistant assists the Dental Director and Dental Hygienist in the care and treatment of patients, filing patient charts, updating records, and performing administrative functions to assist the Patient Assistance Specialist. Must have high school diploma or equivalent. 2-5 years of previous experience in a dental setting preferred.

Staff Psychiatrist/Psychiatric Nurse Practitioner

The Board Certified Staff Psychiatrist or Psych NP works as part of the clinical provider team performing a variety of direct patient services including assessment, diagnosis, evaluation, implementation of treatment plans, and monitoring of follow-up for clinic patients. Physicians also assist in supervision of community health center PA’s or NP’s as appropriate. Successful candidate will have graduated from an accredited medical school with a degree of Doctor of Medicine or Osteopathy, completed an approved residency program in his/her medical specialty, be licensed to practice medicine in the State of Virginia and have the ability to relate effectively with patients and staff. Position is also eligible for NHSC loan repayment.

Licensed Practical Nurse

Individual will function in conjunction with a medical provider, to assist in the provision of professional medical care and nursing support clinical duties in a clinical setting. The nursing staff plays a critical role in the delivery of quality medical care. The LPN is required to have graduated from an accredited Licensed Practical nursing program and be licensed with the Virginia State Board of Nursing with a current LPN license. 1-3 years’ experience in an outpatient clinical setting is preferred. The provider performs these functions under the general supervision of a licensed physician within the context of a federally qualified community health center (FQHC). Patient Assistance Specialist

Position will work at the front desk, and be responsible for greeting and checking in/out patients, verifying all demographic information and insurance. Successful candidate will work closely with nursing staff and providers to assure patient process flow is optimized. Individual will also answer phones, make appointments and handle customer relations in a positive, professional manner. Successful candidate will have a HS diploma or equivalent, Medical Assistant Certification desired, 2-5 years customer service skills preferably working in a medical office, working knowledge of MS Office products (Word, Excel, Outlook),proven ability to work within a team setting, excellent customer service skills with ability to handle all types of customers. Patient Scheduling Specialist

The Patient Scheduling Specialist works as part of the Patient Care Team and assists with the overall scheduling experience. Staff Dentist

The Dentist provides leadership in all areas relating to the dental services at Johnson Health Center’s (JHC). Responsible for recommending and implementing dental policies and procedures; providing dental services and education to the patients of JHC; ensuring continuous quality improvement in dental services; representing JHC at organizations concerned with dentistry services to the underserved; and act as an integral part of the clinical and administrative management team of the health center. Graduation from an accredited dental school. Ability to maintain appropriate clinical privileges required. Must be able to treat both adults and children. Care Management Coordinator

The Care Management Coordinator assists the Care Manager with working with the patient population and other Johnson Health Center (JHC) staff to support a patient-centered model of care.  Under direct supervision of the  Director of Care Management, the Care Management Coordinator assists care managers with identification of patients with complex issues and/or those needing additional screening, as well as providing additional assistance, outreach, and follow-up.

Behavioral Health Therapist (LCSW or LPC)

The Behavioral Health Therapist provides comprehensive assessment, diagnosis, development and implementation of behavioral health treatment plan for JHC patients as referred by JHC medical providers. The behavioral health therapist provides counseling, crisis intervention, various case management services and referral services and works with JHC medical providers to ensure care management of the patient. Position is eligible for student loan repayment through the NHSC. Master’s degree in Social Work with LCSW or LPC, and at least two years of experience required.
