Annual Fund
Your gift to the Annual Fund provides essential, unrestricted support, keeping Johnson Health Center responsive, resilient, and ready. These flexible dollars ensure we can meet urgent needs and deliver care wherever and whenever it’s needed most.
$30 Supports a patient’s medical visits for a full year.

Johnson Health Center provides compassionate care for children and adults through our Patient-Centered Medical Home (PCMH) model. Our team of providers, nurses, and care managers ensures every patient receives high-quality, individualized care.

Transportation – Help Remove Barriers to Care
At Johnson Health Center, we understand transportation is a significant barrier to accessing care for many in our community. To address this, we provide transportation to and from appointments at Johnson Health Center facilities when no other source of transportation is available.
